FOR YOUR FIRST APPOINTMENT, please bring the following items with you:
PAYMENT/COPAYMENT for services is expected at the time of your visit unless you are covered by an insurance plan with which we participate. Payment is accepted in the form of cash, check or credit card.
HELP GETTING HOME - It may be necessary for the doctors to dilate your pupils in order to more thoroughly examine your eye. The dilation could affect your distance vision, so it might be wise to arrange for someone to help you with your drive home if you are traveling by car.
EYEGLASSES - Following your appointment, we invite you to visit Centre Optical Services, which is conveniently located in our State College office, to fill your eyewear prescription. Our friendly staff is available to assist you with your frame selection, lens options, and contact lens concerns.
FREE PARKING:
WHEELCHAIR ACCESS to our State College building is via a ramp on the front of the building facing Locust Lane. Access to this ramp is best achieved from the parking lot and down the sidewalk on Locust Lane and not from the street, as there is a large curb next to the street in front of the ramp.

- Current glasses
- Contact lens information (boxes from current or old lenses are best)

- If you are visiting our State College office, free parking is available in the lot next to our building. Access to our parking lot is best achieved from Clover Alley, which connects to Fairmount Avenue next to Centre Optical Services. Additional parking is available on Fairmount Avenue.
- If you are visiting our Philipsburg office, there is ample parking available in the parking lot directly in front of our office.
